Endemic typhus is transmitted by fleas from rats, cats and opossums. The infection is transmitted by the feces of the infected fleas when it is rubbed into abrasions, exposed to mucus membranes or is inhaled.

I the US the condition is seen most commonly in southern California, Texas, the southeast and Hawaii.
